MEDICAL DIRECTOR
Certifies the attending physicianâs prognosis; leads the hospice team in developing a plan of care; provides consultation to the attending physician.
ATTENDING PHYSICIANâ
Certifies the patient has a terminal illness and remains the patientâs primary physician and works as a member of the hospice team.
CHAPLAIN
Provides spiritual support. Works as a liaison with clergy of the patientâs choice
BEREAVMENT COUNSLOR
Bereavement Counselors offer a variety of services to the patientâs family to help with the understanding of grief
SKILLED NURSES RNâS & LVNâS
Evaluates patientâs condition during regular scheduled visits. The registered nurse coordinates the plan of care and teaches the family proper patient care techniques; the use of medication and management.
HOSPICE VOLUNTEER
Hospice volunteers are kind and compassionate individuals who support patients and loved ones by providing respite care and friendly visits
SOCIAL WORKERâ
Assess social needs of the patientâs family and provides psychological and Bereavement counseling
